1552O Chemotherapy-induced peripheral neuropathy (CIPN) prevention trial evaluating the efficacy of hand-cooling and compression in patients undergoing taxan-based (neo-)adjuvant chemotherapy for primary breast cancer: First results of the prospective, randomized POLAR trial

چکیده

CIPN is a common, dose-limiting side effect of taxane-based chemotherapy. Currently there no established strategy for prevention or treatment. Smaller studies suggest that cooling compression could have preventive effect. In this randomized trial we investigated the effectiveness one-sided hand-cooling CIPN. 122 breast cancer patients who received weekly (nab-)paclitaxel-based (neo-)adjuvant chemotherapy were 1:1 to either dominant hand. No intervention was performed on other Cooling with frozen glove (Elasto-Gel; 84400APT Cedex, Akromed), applied by two surgical gloves (one size smaller than tight-fitting size) 30 minutes before, during and after taxane therapy. The primary endpoint efficacy prevent grade ≥2 sensory polyneuropathy evaluated Common Terminology Criteria Adverse Events v4.0 (CTCAE). second hierarchical test, rates between both groups compared. further assessed Total Neuropathy Score (TNSc), patient self-report questionnaires (EORTC-QLQ-CIPN20), MR-neurography (n=21) nerve conduction studies. Additionally, onycholysis, skin toxicity, quality life, CIPN-associated dose-reductions, treatment discontinuations potential risk factors. highly effective in preventing (cooling: 25 vs. 46%; p-value=0.0008; compression: 23 39%; p-value=0.0016) similar (no significant difference found: p-value=0.7303). sensitive detecting showing increased T2 nerve-to-muscle signal ratio indicating edematous changes. Updated results will be presented. POLAR first compare Both interventions almost halved These findings play an important role beyond gynecological oncology.

A Prospective Triple-blind Randomized Trial on Safety and Efficacy of Abitant in the Prevention of Chemotherapy-Induced Nausea and Vomiting

Background: Oral Aprepitant, a neurokinin-1 receptor antagonist, is suggested in combination with other antiemetic agents in preventing chemotherapy-induced nausea and vomiting(CINV) associated with emetogenic chemotherapeutic regimens in adolescents, but its efficacy and safety in pediatric patients more than six months are unknown. in this study, we used abitant drug (a generic name of Aprepi...

Pathobiology of cancer chemotherapy-induced peripheral neuropathy (CIPN)

Chemotherapy induced peripheral neuropathy (CIPN) is a type of neuropathic pain that is a major dose-limiting side-effect of potentially curative cancer chemotherapy treatment regimens that develops in a "stocking and glove" distribution. When pain is severe, a change to less effective chemotherapy agents may be required, or patients may choose to discontinue treatment. Medications used to alle...

Frequency of Chemotherapy-Induced Peripheral Neuropathy in Cancer Patients in Sari Imam Khomeini Hospital

Background and purpose: Chemotherapy-induced peripheral neuropathy (CIPN) is a debilitating condition that may require reduction or discontinuation of chemotherapy. Identifying effective methods in treatment of established CIPN requires determining the frequency of CIPN. There is a lack of comprehensive studies on this issue, so, this study was conducted to investigate the frequency of periphe...

Mitochondrial Dysfunction in Chemotherapy-Induced Peripheral Neuropathy (CIPN)

The mitochondrial dysfunction has a critical role in several disorders including chemotherapy-induced peripheral neuropathies (CIPN). This is due to a related dysregulation of pathways involving calcium signalling, reactive oxygen species and apoptosis. Vincristine is able to affect calcium movement through the Dorsal Root Ganglia (DRG) neuronal mitochondrial membrane, altering its homeostasis ...

[Neo-adjuvant chemotherapy for breast cancer].

OBJECTIVE To study the value and indications of a neo-adjuvant chemotherapy for breast cancers. METHODS Forty-three breast cancer patients were given the NF neo-adjuvant chemotherapy (Navelbine 25 mg/m2 and 5-Fu 500 mg/m2) for 3 cycles before the operation. Epirubicin (EPI 40 mg/m2) was added in patients with locally advanced lesions. G-CSF 75 micrograms subcutaneous injection was used to inc...
